Hereby, in full consciousness and sanity, not persuaded or forced by anyone, knowing how to read and write, after the legal meaning and all consequences of this act have been explained to me, expressly and without the need for any further questions and/or approval, I give this

by which I, ; , in my capacity as a data subject (hereinafter referred to as the Data Subject) within the meaning of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (hereinafter referred to as the GDPR), expressly confirm that I, as a Data Subject:

• am aware of and agree that the company Marignacija d.o.o. for publishing and services, OIB 54076503328, Tomacova ulica 16, Kuče, City of Velika Gorica, e-mail address info@marignacija.hr contact phone number +385 91 4444 865 (hereinafter Marignacija) processes my personal data for the purpose of my participation in the (hereinafter Spiritual Exercises), and in particular for the purpose of more correct recognition of my spiritual affinities and more efficient participation in the Spiritual Exercises,
• I am familiar with the term personal data and I am aware that this means any data that relates to me as an individual and through which my identity is established or can be established, including, for example, but not exclusively: identification data, contact data, and other data necessary for participation in the Spiritual Exercises,
• I am familiar with the term personal data processing and I am aware that personal data processing means any procedure or set of procedures that are performed on personal data or sets of personal data, whether by automated or non-automated means such as collection, recording, organization, structuring, storage, adaptation or alteration, retrieval, consultation, use, disclosure by transmission, dissemination or otherwise making available, alignment or combination, restriction, erasure or destruction,
• I am aware that data revealing racial or ethnic origin, political opinions, religious or philosophical beliefs or trade union membership, as well as genetic data, bio-metric data for the purpose of uniquely identifying an individual, data concerning health or data concerning an individual's sex life or sexual orientation constitute a special category of data pursuant to Article 9 of the GDPR,
• I am aware of all my rights arising from the GDPR, and in particular:
  • the right to obtain information about the processing of my personal data in accordance with Articles 13 and 14 of the Regulation;
  • the right to access my personal data in accordance with Article 15 of the Regulation;
  • the right to rectify my personal data in accordance with Article 16 of the Regulation;
  • the right to erasure ("right to be forgotten") in accordance with Article 17 of the Regulation;
  • the right to restrict the processing of my personal data in accordance with Article 18 of the Regulation;
  • the right to data portability in accordance with Article 20 of the Regulation;
  • the right to object to the processing of personal data concerning me in accordance with Article 21 of the Regulation;
  • the right not to be subject to a decision based solely on automated processing in accordance with Article 22 of the Regulation;
  • the right to lodge a complaint with a supervisory authority in accordance with Article 77 of the Regulation;
  • on the right to an effective legal remedy against Marignacija in accordance with Article 79 of the Regulation,
• aware that I am authorized at any time, by contacting Marignacija either by e-mail or by post, to withdraw this Consent by a unilateral declaration of will given in writing and that I am not obliged to give this Consent,
• aware of and agree to the transfer of my personal data to third parties with whom Marignacija cooperates in business for the purpose of organizing, conducting and participating in the Spiritual Exercises, for example but not exclusively to Internet providers, business associates who provide administrative support, accounting services, members of the Injigo Community and other persons who are in regular contact with the Injigo Community, and other external associates.

By signing this Consent, I confirm all of the above and give my unequivocal and express consent to the processing of my personal data as stated in this Consent for the purpose of participating in the Spiritual Exercises in accordance with Article 6. paragraph 1 point a) and Article 9 paragraph 2 point a) of the GDPR.

• familiar with the concept of a prayer companion in the Spiritual Exercises, which means a person who communicates with the Respondent on a weekly basis, who has access to a special category of personal data about the Respondent, who assists the Respondent in the process of the Spiritual Exercises, who accompanies the Respondent through the Spiritual Exercises and informs the Respondent about the events in the Spiritual Exercises (hereinafter referred to as the Companion)
• I agree and hereby give my explicit consent for Marignacija to transfer my personal data, including special categories of personal data, to the person who is designated as my Companion in the Spiritual Exercises,
• I am aware that I am authorized at any time, by contacting Marignacija either by e-mail or by mail, with a unilateral declaration of will given in writing, to withdraw this Consent and that I am not obligated to give this Consent.
